Before discharge, ask what care is recommended next and whether the first appointment is confirmed. Write down the location, date, contact person, and what to do if the plan changes. Connect the providers
Ask how discharge information and medication details will reach the next clinician with your consent. Make sure you understand which professional is responsible for prescriptions and follow-up questions. If the receiving program needs an assessment before enrollment, clarify that process before assuming your place is arranged.
Plan the time between appointments
Discuss transportation, living arrangements, and who you can contact for support. Identify situations that could make the transition difficult and ask the team to help plan for them. If you cannot access the recommended care, tell the discharging team so you can discuss alternatives instead of trying to manage the gap alone.
Where outpatient care may fit
Outpatient treatment may be one part of the next phase when it is clinically appropriate. It allows scheduled care while you live outside the program.
